  • You have never been issued a U.S. passport or
  • You are under age 16 or
  • You were under age 16 when your previous passport was issued or
  • Your most recent U.S. passport was issued more than 15 years ago or
  • Your most recent U.S. passport was lost or stolen or
  • Your name has changed since your previous U.S. passport was issued and you are unable to legally document your name change

All Minor applicants, including renewals, must apply in person at a Passport Agency, Acceptance Facility, U.S. Embassy or Consulate. Please see Special Requirements for Children Under Age 16.

One parent or Guardian applying for a passport

  • You are the non-applying parent or guardian providing written consent for your minor child to apply for a U.S. passport (because you are unable to appear in person with your child) or
  • You are the applying parent or guardian, and the written consent of the non-applying parent or guardian cannot be obtained
  • Then fill out this DS-3053 form.

Form DS-3053 must be submitted with Form DS-11 at an Acceptance Facility or Passport Agency.

IMPORTANT NOTE: The notarized Form DS-3053 must not be more than 3 months old. The applying parent will need to submit a new DS-3053 as written consent from the non-applying parent with all future applications.
